Support at Home - Helping You Live Well, Your Way

From 1 November 2025, the Support at Home program will replace the current Home Care Packages system.

At Alpha Omega Aged Care, we'll walk alongside you through these changes, making sure you understand your options and can keep living safely, comfortably, and independently at home.

Quick Facts

  • 8 funding levels instead of 4 – more tailored to your needs
  • Quarterly budgets set by the Australian Government
  • Three service categories: Everyday Living, Independence Support, Clinical Care
  • Contributions based on your income and the type of service
  • One assessment process for all aged care services
  • Hardship assistance available if you can’t afford contributions
  • Includes palliative and end‑of‑life care options at home

What Is Support at Home?

Support at Home is a new national program that will bring together:

  • Home Care Packages (HCP)
  • Short‑Term Restorative Care (STRC)
  • Later, the Commonwealth Home Support Programme (CHSP)

Why the change?

  • To make funding fairer and more consistent
  • To simplify the system into one program
  • To give you more choice and flexibility in the services you receive

How Funding Works

  • After your assessment, you'll be placed into one of eight classifications.
  • Each classification has a fixed quarterly budget set by the Australian Government.
  • Budgets are not negotiable but can be reviewed if your needs change.
  • Unspent funds can roll over to the next quarter, but only up to $1,000 or 10% of your budget (whichever is greater).
  • Funding can be used across three service categories:
    Category Contribution Testing Description
Clinical Care No contribution – fully funded Assessed by Services Australia Nursing, allied health, medical support
Independence Support Moderate contribution Income and asset test applies Personal care, mobility aids, home modifications
Everyday Living Higher contribution Based on financial situation Domestic help, transport, meals

Note: Hardship assistance does not require means testing

Care Management Services

Your funding includes care management — the behind‑the‑scenes work that keeps your care running smoothly.
What we do as your care manager:

  • Develop and regularly review your care plan
  • Coordinate services and schedule visits
  • Monitor your budget and spending
  • Adjust services as your needs change
  • Liaise with health professionals and family members
  • Ensure your care meets quality and safety standards

Fee cap: Under Support at Home, care management fees are capped at 10% of your budget.

The Role of Your Care Partner

At Alpha Omega Aged Care, your Care Partner is your main point of contact and advocate.
They will:

  • Listen to your goals and preferences
  • Help you navigate the aged care system
  • Coordinate with service providers on your behalf
  • Keep you informed about your budget and services
  • Support you in making changes as your needs evolve

Think of your Care Partner as your personal guide — making sure your care is not just clinically right, but personally right.
